Care Connection for Children is a partner organization in
the national effort to accomplish the goals of Healthy People 2020: an action plan to achieve community-based service systems
for children and youth with special health care needs and their
families. In addition to community-based, the service
system needs to be comprehensive, coordinated, culturally competent
and family-centered.
This
plan is a part of Healthy People 2020 from the U.S. Department
of Health and Human Services.
State,
community
and public and private sectors are partnering to achieve the
goal to “bring better health to all people in this country.”The
Maternal and Child Health Bureau partnering with Family Voices,
the March of Dimes, the American Academy of Pediatrics
and other organization have identified the following outcomes:
GOALS
All children and youth residing in Central Virginia who have special health care needs will receive family centered, community-based, culturally sensitive, comprehensive multi-disciplinary specialty services. -
Central Virginia Care Connection for Children will strive to accomplish the federal Maternal and Child Health Bureau’s performance measures for children and youth with special health care needs (CYSHCN).
Core Outcomes to be achieved (based on MCHB performance measures and Healthy People 2020 Goals relevant to CYSHCN):
All children with special health care needs will receive coordinated ongoing comprehensive care within a medical home. All families of children with special health care needs will have adequate private and/or public insurance to pay for the services they need. All children will be screened early and continuously for special health care needs. Services for children with special health care needs and their families will be organized in ways that families can use them easily. Families of children with special health care needs will partner in decision making at all levels, and will be satisfied with the services they receive. All youth with special health care needs will receive the services necessary to make appropriate transitions to adult health care, work, and independence.

by these children and their families.
The American Academy of Pediatrics
The mission of the American Academy of Pediatrics is to
attain optimal physical, mental and social health and well-being
for all infants, children, adolescents and young adults.
To this
purpose, the AAP and its members dedicate their efforts
and
resources.
March of Dimes
Four major problems threaten the health of America's babies:
birth defects, infant mortality, low birth-weight, and
lack of prenatal care. The March of Dimes is committed
to the
day when all babies will be born healthy.
